Hospital Provincial Clinicoquirúrgico Docente "Saturnino Lora"

Se realizó un estudio descriptivo, transversal y prospectivo de 29 pacientes en quienes se diagnosticó un enfisema orbitario, seleccionados de un total de 89 que fueron examinados en el Servicio de Cirugía Maxilofacial de Urgencia del Hospital Provincial Docente “Saturnino Lora” de Santiago de Cuba por haber sufrido algún traumatismo en la región del tercio medio superior de la cara durante el cuatrienio 2003 - 2006. A dichos lesionados se les efectuaron rigurosos exámenes (clínico e imagenológico), tanto en el mencionado Servicio como en la sala, y sobre la base de los hallazgos en ambas pruebas, todos pasaron a ser reconocidos por especialistas en oftalmología. El enfisema orbitario es una complicación de las fracturas orbitarias, no tan infrecuente como se considera, de carácter autolimitado, cuya aparición deviene un marcador de lesiones subyacentes más graves. La tomografía axial computarizada, en unión del reconocimiento clínico, constituyen los indicadores más seguros de la imperiosidad de operar tempranamente para poner al descubierto o tratar una disfunción mecánica ocular.

A descriptive, cross-sectional and prospective study was carried out in 29 patients with a diagnosis of orbital emphysema, who were selected from a total of 89 patients examined at the Emergency Maxillofacial Surgery Service of the "Saturnino Lora" Teaching Provincial Hospital in Santiago de Cuba for having suffered some injury in the upper midface during 2003 - 2006. These injured patients underwent rigorous tests (clinical and imagenological) in both the aforementioned service and the ward; and according to findings of both tests, all the patients were examined by ophthalmology specialists. The orbital emphysema is a complication of orbital fractures; it is not as uncommon as it is considered, and its character is selflimited, which occurrence becomes a marker of more severe underlying injuries. The computerized axial tomography and clinical examination constitute the surest indicators for early surgical intervention with the purpose of detecting or treating an ocular mechanical disfunction.
